Important info that you should know before you "Submit":
1) Entries in multiple fields are treated as an "OR"
2) UPPER/lower case chars are considered as equals  
3) Multiple words must match EXACTLY as entered in  
   each field, so 'Cape Cod' isn't 'Cape' or 'Cod'
4) Default is current month + up to two prior months
5) Use '99' for month to scan all available months
